Need honest advice on CCS case pacing, clock advancement, and 2-min counseling, keep second guessing myself

Sorry if my english is little bit rough today, been studying 9 hrs straight. I am 4 weeks out and CCS case pacing, clock advancement, and 2-min counseling is still my lowest bar on practice forms. Whenever they test placing stat pulse ox, monitor, IV access, BP cuff before full physical exam in unstable ED pts alongside advancing clock w/ "next available result" instead of jumping 24 hrs + adding VACCINE/COUNSEL orders at 2-min screen, I narrow it down to two choices and pick the wrong one almost every time.

Most of my misses happen bc of forgetting urine beta-hCG in reproductive-age female before abdominal CT or surgery on CCS. Anyone got a simple rule of thumb for this?

Context: currently while reviewing my 3rd nbme form after missing two similar stems.

Honestly the trick that saved me on CCS case pacing, clock advancement, and 2-min counseling while reviewing my 3rd nbme form was focusing 100% on stopping the clock as soon as diagnostic lab/imaging returns to start definitive tx and cancel empiric orders. Dont try to memorize a massive table while you are tired after missing two similar stems.

Instead, split the stem into two quick checks: (1) is placing stat pulse ox, monitor, IV access, BP cuff before full physical exam in unstable ED pts present, and (2) how does advancing clock w/ "next available result" instead of jumping 24 hrs + adding VACCINE/COUNSEL orders at 2-min screen change the next step? 90% of the time people miss this bc of forgetting urine beta-hCG in reproductive-age female before abdominal CT or surgery on CCS. Write a 1-line contrast note on an index card and read it before your morning block tomorrow, it clicks super fast.

Can confirm as a PGY-1 now, stopping the clock as soon as diagnostic lab/imaging returns to start definitive tx and cancel empiric orders is not just high yield for the exam on CCS case pacing, clock advancement, and 2-min counseling while reviewing my 3rd nbme form, you literally use placing stat pulse ox, monitor, IV access, BP cuff before full physical exam in unstable ED pts on the wards all the time after missing two similar stems.
