Missed 4 qs in a row on coagulation cascade and bleeding disorders this morning, lowkey panicking

Ngl I thought I had coagulation cascade and bleeding disorders down cold after my first pass of qbank, but my last two blocks proved me completely wrong lol. The stems keep testing PT/INR extrinsic Factor VII vs aPTT intrinsic Factors VIII, IX, XI, XII and von Willebrand disease prolonged bleeding time + elevated aPTT bc vWF carries Factor VIII in super subtle ways.

How are you guys spotting checking platelet count + mucosal bleeding time vs deep hemarthrosis first fast enough on timed mode without getting baited by forgetting Bernard-Soulier has big platelets (GpIb defect) while Glanzmann fails ADP aggregation (GpIIb/IIIa)?

Context: currently in my morning study group session going over high-yield nbme concepts.

The conversation

I had the exact same issue w/ coagulation cascade and bleeding disorders when I was in my morning study group session. What finally fixed it for me is checking platelet count + mucosal bleeding time vs deep hemarthrosis first.

Whenever you see PT/INR extrinsic Factor VII vs aPTT intrinsic Factors VIII, IX, XI, XII in the stem, pause for two seconds and look specifically for von Willebrand disease prolonged bleeding time + elevated aPTT bc vWF carries Factor VIII before peeking at the answer choices. That way forgetting Bernard-Soulier has big platelets (GpIb defect) while Glanzmann fails ADP aggregation (GpIIb/IIIa) cannot bait you going over high-yield nbme concepts. Once I started doing that on every block, coagulation cascade and bleeding disorders went from my worst section to basically free points.

That point about avoiding forgetting Bernard-Soulier has big platelets (GpIb defect) while Glanzmann fails ADP aggregation (GpIIb/IIIa) is huge ngl. I got burned by that exact distractor in my morning study group session going over high-yield nbme concepts until I started checking von Willebrand disease prolonged bleeding time + elevated aPTT bc vWF carries Factor VIII first.

If you are in my morning study group session going over high-yield nbme concepts, dont beat yourself up over coagulation cascade and bleeding disorders misses rn — missing von Willebrand disease prolonged bleeding time + elevated aPTT bc vWF carries Factor VIII on qbank today means you will spot it immediately on exam day.

Adding onto what was said above: for coagulation cascade and bleeding disorders, if you ever get stuck b/w two choices in my morning study group session going over high-yield nbme concepts, go back to checking platelet count + mucosal bleeding time vs deep hemarthrosis first and pick the option that addresses PT/INR extrinsic Factor VII vs aPTT intrinsic Factors VIII, IX, XI, XII directly.
